You can reassign a patient's care team directly from the patient list.
Steps
Open your patient list and find the patient.
Hover the patient's row and choose Edit Patient Care Team from the row actions.
In the Update Care Team dialog, set the Primary Provider (the provider responsible for the patient, who receives billing notifications), and add or remove Secondary Providers as needed.
Click Update.
Moving a patient to a provider at another of your locations
If the provider works at a different location in your practice, you do not need to add that location to your own account first. Owners can move a patient to any active location in the practice in one step.
Open your patient list and find the patient.
Hover the patient's row and choose Move to another location from the row actions.
Pick the location, then the provider there who becomes the patient's Primary Provider.
Click Move.
The patient's care clinic and care team update together. If you are not a member of the new location, the patient leaves your own patient list; the receiving provider sees them right away. Clinic admins see only the locations they administer.
The new provider isn't in the picker, or you get an error?
The provider picker only lists providers who have an active SaRA account at your organization and at the patient's location. If the provider you're moving the patient to isn't listed, or you see an error saying the provider must be enrolled or active, they don't have an active SaRA account yet, or they work at another location (use Move to another location above). Get them set up in SaRA first, then reassign the patient. If you're not sure of their account status, message support with the provider's name and email.
A provider is leaving your practice?
Reassign their patients to another provider before deactivating their account: a user who still has active patients assigned can't be deactivated. If you have many patients to move, contact support and we can help with a bulk reassignment.
