Reviewing your claim form (the CMS-1500)

Written by Valeed Malik

When we prepare claims for your practice, we send you a completed 1500 claim form to review before anything is submitted.

You're welcome to review the whole form, and we ask you to look closely at three boxes:

  • Box 25: your federal tax ID

  • Box 32: the service facility (where care was provided)

  • Box 33: the billing provider (who gets paid)

You review and approve the form in SaRA: open Documents in the left menu and the form is waiting there. If anything needs a correction, mark it as needing a change on that page and we update it before anything goes out.

Nothing is submitted until you approve the form in Documents. If the form sits unapproved, your claims wait with it, so an approval the same week keeps your first claims on schedule.
