Where this comes up
When you re-activate an inactive patient (Patient list > Inactive tab > Activate RTM), SaRA asks: "Is this a new plan of care?" Your answer controls how SaRA treats the new RTM episode for billing, so it is worth getting right.
Choose Yes: a new episode of care
Select Yes when the patient is starting a new episode of care, for example:
Returning after discharge or a long gap in care
A new injury, or care that has moved to a different body region after a new evaluation
A new evaluation establishing a new plan of care
SaRA starts a fresh RTM episode, and the initial setup and education code (98975) becomes billable again once the monitoring requirements are met — see RTM CPT codes & billing thresholds (2026).
Wrong body part picked at enrollment? That is a correction, not a new plan of care, and it does not go through this question. While the patient is still active, change the Patient type from the patient's row actions under Update RTM Status. The patient stays enrolled, the current cycle continues, and the check-in questions switch from the next scheduled message. Steps: Changing a patient's type after enrollment.
Choose No: continuing the same plan of care
Select No when the patient is continuing the same plan of care, for example they were paused, briefly unenrolled, or texted STOP and then START. SaRA continues the prior episode: monthly monitoring codes bill as normal, but no new 98975, because 98975 bills once per episode of care.
Why it matters
Billing 98975 twice in the same episode creates compliance risk. Missing it on a genuine new episode leaves earned revenue uncaptured. Neither answer is "safe by default" -- answer based on whether a new episode of care is actually starting.

Message us through in-app support. We can correct the enrollment on the back end without re-enrolling the patient and without interrupting their monitoring days -- there is nothing you need to redo in the app.
