SaRA billing runs beside your EMR and your clearinghouse, not instead of them.
When SaRA bills RTM (remote therapeutic monitoring) for you, your practice keeps its EMR, keeps its clearinghouse, and keeps its biller. SaRA adds one narrow lane for the RTM claims. This page explains what that lane needs from your setup, so you can see where you fit before anything is set up.
Your EMR: what connects automatically
SaRA reads patient information from your EMR so your team is not retyping it. How much comes across depends on the EMR.
Full connection | Raintree, Prompt, Athelas, WebPT, Athena, SPRY | Patient identity, demographics, and insurance come across on their own. Appointments come across for Raintree, Prompt, Athelas, and SPRY. |
Identity connection | Empower, HelloNote, Heno, and other EMRs that work through the SaRA browser extension | Patient name, date of birth, and EMR ID come across when your team opens the chart. Empower also sends address and phone. Anything that does not come across is entered in SaRA by your team. |
No connection yet | Any EMR not listed above | In most cases SaRA billing is not a fit yet. The exception is a practice whose team enters every patient's details in SaRA by hand and whose biller uses Waystar. Tell us your EMR and we will say which side you land on. |
Not sure which group you are in? Ask us in the in-app chat and we will tell you before you commit to anything.
Your clearinghouse: two rules that decide everything
A clearinghouse is the service that carries claims from a practice to the insurance payers and carries the payment reports (remittances) back. SaRA sends RTM claims through its own clearinghouse account. Two rules explain how that fits with yours.
Rule 1: claims can go out through more than one clearinghouse. Your biller keeps sending your regular claims through your clearinghouse. SaRA sends only the RTM claims through ours. Nothing about your biller's claims changes.
Rule 2: payment reports only flow to one place. Each payer sends its remittance files to one clearinghouse per practice. For most practices that is the one your biller already uses, and SaRA does not move it.
The one thing that could disrupt your biller is a payer enrollment that moves your payment reports to a new clearinghouse. SaRA's enrollment forms ask payers for claims only, never for remittances, so your biller keeps receiving every payment report exactly as today.
What that means for your practice
You use Waystar | The smoothest fit. With your Waystar account name and number, we copy your existing payer enrollments instead of asking you to sign new forms, and your RTM payment reports route to us automatically. Your biller does nothing new. If your EMR is also in the full connection group above, that is an automatic yes. The one thing we ask is that you tell us right away if you ever change your EMR or your clearinghouse, so your claims and payment reports keep flowing. |
You use another clearinghouse (Office Ally, Availity, Claim.MD, Jopari, Inovalon, and others) | Billing works, with two things on your side. Some payers, Medicare especially, need a one time claims-only enrollment so our clearinghouse can send claims under your NPI (your practice's provider number). We prepare the form, you print, sign, and return it, and we confirm the timeline during setup. Because the RTM payment reports land with your biller, we also need a way to see them for the RTM payments on a short list we provide. Your biller either uploads those remittance files into SaRA or gives us a login to your clearinghouse so we can pull them ourselves. If your biller uses Jopari, most RTM payment reports reach us on their own and we only ask for the few that do not. This path needs an EMR from one of the two connected groups above. |
You use Prompt with Office Ally | Billing works. The RTM payment reports show up in Prompt but cannot be exported from it in bulk, so your biller either sends us the remittance files from Office Ally or gives us access to the billing tab in Prompt, where we pull the payment reports ourselves. We agree on which of the two before anything is set up. |
Your EMR is also your clearinghouse (Tebra and similar) | Billing works, with the same claims-only enrollment and one extra step. Your team exports the payment reports from the EMR and uploads them into SaRA. Tell us which system it is and we will confirm what that export looks like. |
You bill on paper with no clearinghouse | SaRA billing is not a fit today. If you move to a clearinghouse later, tell us and we will take another look. |
No visit gets billed twice
SaRA is the only one sending the RTM codes (the ones starting 989) when SaRA bills for you. Your biller leaves those codes alone, and everything else stays theirs. If you ever move RTM billing back to your biller, we hand over the list of everything we submitted so the same visit is never billed twice.
What we will ask you for
Which EMR you use, and a login for the connection where one applies.
Which clearinghouse your biller uses. If it is Waystar, the account name and number.
Your top payers, so we can tell you up front which ones need an enrollment form.
An introduction to whoever runs your billing, so the payment report step is set up with them directly and you are not in the middle.
A heads up if you change your EMR or clearinghouse at any point, before the switch if you can.
The two billing paths themselves are described in Choosing your billing path.
