Your first billable cycle, explained

What happens between enrolling your first patient and your first billable RTM codes, and how long it takes

Written by Valeed Malik

From enrollment to billable codes in one 30-day cycle.

You've enrolled your first patient. This is the path from that moment to the first codes you can bill, and what SaRA is doing at each point along the way.

Day 0: enrollment starts the clock

When you enroll a patient, SaRA sends their first text the same day, with consent built into that first message. A 30-day monitoring cycle starts.

If you haven't enrolled anyone yet, start with Enroll a patient in SaRA. What that first message looks like, and how consent works, is in The first text your patient receives.

Days 1 to 30: replies do the work

SaRA checks in with your patient by text about their home exercises. Every calendar day the patient responds counts as one day of engagement, and days of engagement are what the billing thresholds run on.

You can watch it happen from your patient list. Each patient's row shows where they are in the current cycle and how their engagement is tracking.

Nothing to install and nothing for the patient to log in to. If your patient replies to their texts, the days accumulate on their own. Column-by-column detail lives in Reading your patient list.

The thresholds, in plain terms

At the end of the cycle, the monitoring code your patient earned depends on how many of the 30 days they engaged. Sixteen or more days earns the standard monthly monitoring code. Two to fifteen days earns the lower-threshold code that was added in 2026. SaRA works this out for you and updates it as days come in.

The first cycle also carries a one-time setup and patient education code. It becomes billable when that first cycle ends and has earned a monitoring code.

The exact codes and their criteria live in RTM CPT codes & billing thresholds (2026).

Your team's time counts too

Alongside the day counts, the time your team spends reviewing data, messaging the patient, and logging interactions accrues toward the treatment management codes. Those run by calendar month rather than by cycle, and SaRA tracks the minutes for you. The details are in How SaRA tracks your RTM time.

Day 30: the cycle closes and codes appear

When the cycle ends, what it earned shows up on your Billing page. If you self-bill, the item lands in Ready to Bill with the codes and justification text ready to hand to your biller. If SaRA bills for you, we take it from sign-off to submission.

Cycles then run back to back. The next 30-day cycle starts the day after the first one ends, for as long as the patient stays enrolled.
