Which words stop a patient's texts, and how they turn them back on

The exact words that unsubscribe a patient by text, what happens right after, what START does and does not do, and the better option of deactivating in SaRA instead.

Written by Valeed Malik

A patient can end their SaRA texts by replying with a single word. Here is which words count, what happens next, and how the patient turns messages back on.

The words that stop messages

The reply has to be the word on its own. Capital letters and extra spaces do not matter.

  • English: STOP, STOPALL, UNSUBSCRIBE, CANCEL, QUIT, OPTOUT, REVOKE

  • Spanish: ALTO, PARADA

  • Russian: СТОП

A longer message that happens to include one of these words does not count. "Please stop texting me so early" is treated as a normal reply and lands in the patient's conversation like any other. A reply of END is an ordinary answer, not a stop word.

A check-in answer can never be one of these words. If you ever write a message whose answer choices include one of them, a patient who replies with that word will be unsubscribed, not answered.

What happens right after a stop word

  • Automated check-ins stop right away.

  • The patient's RTM enrollment ends. SaRA records the reason as the patient opting out by text, so it shows up that way in your patient list and reports.

  • A note appears in that patient's conversation in SaRA saying they replied STOP and will need to text START to reopen the line.

  • Texts you send from SaRA will not reach the patient until they text START. Only the patient can reverse a stop word. Nobody at your clinic, and nobody at SaRA, can do it for them.

What START does, and what it does not do

These words turn messages back on. Again, the word has to be the whole reply.

  • English: START, RESUBSCRIBE, UNSTOP

  • Spanish: COMIENZA

  • Russian: НАЧИНАТЬ, НАЧИНАЮ

START reopens the line. You can message the patient again and they can message you.

START does not restart check-ins. Putting a patient back on RTM is a decision for the care team, so SaRA never re-enrolls anyone on its own. Instead, if the patient was in your care within the last six months, an Opted Back In card appears at the top of your patient list so you can decide. The card is explained in The Opted Back In card on your patient list.

To resume check-ins, open the patient list, go to the Inactive tab, open the patient's row actions and choose Activate RTM. SaRA will ask whether this is a new plan of care. If the patient is continuing the same plan, answer No. The full guidance is in Re-activating a patient: continuing vs. starting a new plan of care.

A better option than telling a patient to reply STOP

If a patient wants fewer messages, or is being discharged, deactivate them in SaRA instead. That stops the automated texts but keeps the line open in both directions, and you can re-activate them later without the patient having to do anything. The steps are in Discharging a patient and stopping their automated messages.
