What your billing company or clearinghouse needs when SaRA files your RTM claims

When SaRA bills for you, RTM claims leave through SaRA's own clearinghouse account while your biller keeps everything else. What to enter in Billing setup, what to tell your billing partner, and which enrollment is ours.

When SaRA bills for you, your RTM claims leave through SaRA's own clearinghouse account. Your billing company and your own clearinghouse keep doing everything else. Here is what each side needs so the two never collide.

How RTM claims travel when SaRA bills

SaRA prepares each RTM claim after you sign off on it, then files it with the payer through SaRA's clearinghouse, Waystar. Your visit claims, your other codes, and your existing payer relationships do not move. Your billing company or clearinghouse keeps sending those exactly as today.

If you are not sure which billing path you are on, Choosing your billing path shows how to tell from your Billing page.

What SaRA needs from your clinic

Everything on this list is entered once, by the practice owner, on the Manage page under the Billing setup tab. The page and each field are explained in Billing setup: what we need from you and where to enter it.

  • Your EHR and your clearinghouse. If you are not sure which clearinghouse you use, your billing company knows.

  • Your own Waystar account name and number, if Waystar is your clearinghouse. Your payer enrollments already live there. With these two details our billing team copies them into SaRA's account, so most payers need no new paperwork from you.

  • A billing contact. Name, role, email, and whether they can sign payer enrollment forms. This is often someone at your billing company rather than the owner, and that is fine.

  • Your Medicare PTAN and how your practice bills (independent practice, hospital outpatient, or CORF).

  • Two logins in your EHR for SaRA's billers. The page shows the email to use for each.

Our billing team cannot start payer enrollment until the Waystar account (if you use Waystar) and the billing contact are filled in. If either is blank, you will hear from us asking for it.

What to tell your billing company or clearinghouse partner

  1. RTM codes for SaRA-enrolled patients are billed by SaRA. Ask them not to add 98975, 98977, 98985, 98979, 98980 or 98981 to visit claims for those patients. A code billed from both sides is a duplicate, and the payer pays one and denies the other.

  2. Agree on a cutover date of service. Dates before it belong to your biller. Dates on or after it belong to SaRA. Write it down where both teams can see it.

  3. Loop us in on anything RTM-related that lands with them. If a payer sends your billing company a letter or a portal message about RTM claims, tell us through the in-app chat with the patient's initials and the date of service.

Two different enrollments, and which one is ours

Payer enrollment gets a payer to accept RTM claims from SaRA on your behalf. Some payers, Medicare among them, require it before they will pay. SaRA prepares that paperwork, posts it to your Documents page, and files it once it is signed. The four stages and who acts at each are in Payer enrollment for RTM claims.

Remit enrollment (ERA and EFT) is separate. SaRA does not handle it, and it stays with your practice or your clearinghouse. A payer sends electronic payment detail to one clearinghouse per practice, so if your ERAs already point at your clearinghouse, payment detail for RTM claims lands there too. If payment detail for an RTM claim arrives on your side and you do not see it reflected in SaRA, tell us through the in-app chat with the patient's initials and the date of service, and we will match it to the claim.
